Drop In/Beginner Class $15/Person

Release and Waiver:

I understand and agree that participating in any dance class, workshop, rehearsal, or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me during any dance class, workshop, rehearsal, or activities that IV Motion LLC hosts at any studio or location. I also exempt, release, and indemnify IV Motion LLC, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, or property which may arise out of or in connection with participation in any classes or activities conducted by IV Motion LLC. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold IV Motion LLC, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I have any questions or concerns about whether or not a particular activity is appropriate to my current health status, I understand it is my responsibility to ask my doctor if this activity is appropriate before I participate in such activity. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. 

I understand this is an adult class and there may be explicit content such as music or movement, if ever I feel uncomfortable I am welcome to leave; no refund will be issued if so. IV Motion LLC reserves the right to refuse services.

I also voluntarily agree, and therefore grant permission for IV Motion LLC to use photographs or videos of me for publicity purposes, and will not hold IV Motion LLC liable. 

I understand and agree that participating in any dance class, workshop, rehearsal, or performance, there is a possibility of exposure and any and all risks related to Covid-19 and variants. While particular rules and personal discipline may reduce risk, the risk of serious illness and death does exist. 

I voluntarily agree, therefore, to assume all risks and responsibility for any claims related to Covid-19 and other infectious diseases, which might occur to me or my child during any dance class, workshop, rehearsal, or activities that IV Motion LLC hosts at any studio or location. 

I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold IV Motion LLC, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such risks, illness or death. 

I, for myself and on behalf of my heirs, assigns, personal representatives and rest of kin, hereby also exempt, release, hold harmless and indemnify IV Motion LLC, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students with respect to any and all illness, disability, death or loss or damage to person or property, whether arising from negligence or releases or otherwise.

I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious disease.

If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.  

If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. 

I have read, understood and agree to be bound by the above statement and consent to all terms above with the purchase of services from IV Motion LLC.
